The Appointment Request feature is only to be used to schedule non-urgent appointments only. If you need to schedule an urgent office appointment or need to reschedule or cancel an office appointment, please call our office.
If you already have a Patient Portal account with us, please log in to view your appointments. If you do not have a Patient Portal account, you will be registered online once you make this appointment. Once your appointment is scheduled, you will receive an email to log in to the Patient Portal to view your upcoming appointment date and time and update and complete your demographics. You must respond to this email to confirm your reserved appointment date in time. No response within 2 days will lead to your reservation being cancelled.
