Chronic Care Management

Urology Associates is now offering our patients an opportunity to enroll in a new program developed by Medicare called Chronic Care Management (CCM).  The service is furnished by our dedicated, trained coordinators who review the status of each patient’s medical record at least once a month, reach out by telephone to assess your condition and interact with your Urologist for further recommendations regarding your care plan with the interest of improving your overall care.  Patients who enroll have access to the practice 24/7 including a dedicated CCM phone line.

These services are covered by Medicare Part B and many managed care plans.  Often, there is little or no co-pay for these services.
